Montlake Start of School Forms and Information
Welcome Montlake Families!
We are looking forward to a great year with you. Below is a link to the first day packet of district required forms. Almost all of them can be completed online and returned electronically. Except for one, the FERPA form needs to be printed, completed and signed.
To complete the required FERPA form please print off or request a printed copy and we can send it home with your student. Please complete and sign the form and return it in person (sending it in your student’s backpack is fine) or by mail to:

Electronically complete the forms listed that apply to your student(s) and submit them to Montlake.email@example.com
List of forms to complete:
- Emergency Information and Student Release Form
- FERPA (this form needs to be printed out, signed and returned in person or by mail, instructions above).
- Survey to Identify Disabled Students ONLY if you think your child has a disability and is NOT currently eligible for Special Education services.
- McKinney Vento – return ONLY if you do NOT rent or own your house/apartment.

SchoolPay Donation – Montlake has ordered the students general supplies needed for school, parents are asked to make a donation via Schoolpay for $15. Visit the link above to make a donation. Thank you!!
We are grateful for any and all donations. This money is allocated to cover expenses for basic school supplies only. Should there be any surplus funds the money will roll forward into next years budget to be used for student supplies.

SchoolPay is the online payment service that SPS schools use for parent payments. You can find out more information and register on the SPS website.
Please note there is a new parent support help number for Schoolpay! School/Parent/Guardians can receive support by calling 1-833-731-2600.
Digital Classroom Subscriptions
Each year, caregivers are asked to pay fees for each child that will help to pay for digital and paper subscriptions. This year, we are asking for an additional fee to pay for covid safety supplies (yoga mats for students to use when eating lunch outdoors). The PTA will subsidize the remainder of these costs.
The fee per student is $20. If this fee is a hardship for your family, you have the option to pay what you are able. All students will have access to subscriptions and supplies, regardless of what they pay.
Once you’ve paid your fees, you will see a volunteer survey. Please complete the survey to indicate if and how you are able to volunteer this year.
